The appointment form collects your name, phone number, reason for visiting and an optional message. This information is used to contact you and organise your request. Avoid sending medical reports or detailed health information. Submitting a request does not confirm an appointment; the centre contacts you to confirm.

Processing appointment requests

The form prepares a WhatsApp message that you must send yourself. WhatsApp processes it under its policy. The centre may use Google Sheets if server-side request processing is enabled. Hosting services process technical data needed to operate the website. When enabled, Upstash uses IP addresses to limit abusive submissions.

Cookies and external services

Technical cookies may protect the contact form or allow access to a preview version. WhatsApp and Google Maps links open external services governed by their own privacy policies.
